Documents Needed:

• Birth certificate

• Social Security card

• Proof of residency (electric or water bill)

• Transcript or last report card*

• Parent/guardian driver's license or state ID

• Any special program documents (LPAC, SPED, 504)

• Immunization records

• State test scores

*If the report card is from a middle school, it must indicate the student has been promoted to ninth grade
